3. Ordering Additional Trophies 

Should you wish to order additional trophies for your winning catalyst, you will need to reach out and email the vendor direct who can then process your order.

When placing your order please complete the attached below spreadsheet and IOR form and send to Sharron Saunders – [email protected] (Account Manager, Special EFX Ltd)

PLEASE NOTE 

• Please contact the vendor before 30th November.
• Customer/Member is responsible for all costs and payment is due at time of order – no trophy will be made until full payment is received.
• There may be duty/ taxes to pay to receive this award which you would need to pay directly with the courier before receiving the award.
• Trophies cannot be customised they will be a duplicate of the trophy from Innovate Asia 2024
• TM Forum will not be involved in the order process, all questions and queries are to be taken up with the vendor directly
• Specify the Name of Catalyst Award Category (e.g. Outstanding Catalyst - Business Impact) in the columns highlighted in colours.
• Please complete all fields on the spreadsheet. We need the full legal company name and must have a contact name, email and telephone number.

Pricing

The price for 1 award will be £255.00 (not including Carriage or VAT), once you contact EFX with the completed form we can estimate carriage and send your invoice for payment.

Providing payment has been received, duplicate orders take approximately 8 weeks. All duplicate order requests will be placed together and start production shortly after the deadline  

If sending outside UK

-  The Importer of Record Agreement (IOR) form will need to be completed.
Completion of this form is essential to avoid lengthy processing delays or even refusal of entry by the local customs authority.
If this information is not provided, please be aware EFX cannot be held responsible for any customs delay and consequential effects on the delivery time.
